The Australian Commission on Safety and Quality in Health Care (the Commission) collects personal information when the information is necessary for, or directly related to, the Commission's functions or activities.
Your personal information is being requested for the Commission to fulfill its function as a provider of Quality Use of Medicines training. The personal information is required for the Commission to create an account and enable you to access the Commission's learning activities. It is also used to enable your organisation to know which learning activities you have started and completed. If you do not want your organisation to see your participation information, please select the "Exclude me from any learning completion reports provided to the organisation selected above". This can also be done in your profile.
If you do not provide the requested personal information, the Commission is unable to create an account on this Learning Management System.
All personal information is held securely. The Commission treats all personal information in accordance with the Australian Privacy Principles under the Privacy Act 1988 (the Act) and the Commission's Privacy Policy. The Commission does not disclose personal information to other organisations outside of your selected organisation unless it is required to do so under the Australian law or tribunal/court order. The Commission's Privacy Policy is available from the Commission's website at www.safetyandquality.gov.au.
